This is the 3rd request for "feedback"/survey.
Right after my visit, I received an email request for evaluation of service. I responded with my feedback. Very soon after that, I received a text message requesting the same thing. I did not respond to that request.
Now I just received this request via email.
Please check your system to determine why I has asked 3 times.
These days we receive so many request forms from not only doctors but others that they have become a nuisance.
